If your ISP allow you to poll the equipment using SNMP instead of sending SNMP traps that would be better. This way the connection would originate from inside the network, so it would not be an opened door on the Internet... If you need minimal monitoring (up/down), you can simply setup an ICMP poll in NPM that would send a ping at a specified interval, if the external routers allows and respond to it.
